Withdrawing from the Service

Can I change my mind about the Service?

Yes, Stay Well at Home is provided with the consent of both the person and their family. The service can be removed should you change your mind. To arrange this, you can contact the HOWZ support service number or your local Memory Assessment Service.

Do I have to receive the Service?

No. It is entirely up to you as to whether you would like to receive the Service. It is an addition to your usual care, which will not be affected.  If, at any time, you decide not to receive the Service your decision will in no way compromise your rights and the standard of care you will receive. You do not have to receive this Service. If you do decide to have the Service, you can stop at any time and do not have to provide a reason.
