“Rachel has made an outstanding contribution since joining ABUHB earlier this year. She has confidently established a brand-new pharmacy service in the GUH discharge lounge, working independently within a multi-disciplinary team. Her clinical interventions at discharge have enhanced patient safety and continuity of care. Through effective counselling, she supports patients transitioning from hospital to community, improving medication adherence and reducing readmissions.
“Rachel actively signposts services such as the common ailments scheme, DMR, and smoking cessation, promoting wellbeing and care closer to home. She ensures efficient medication transfer in the step-down model, reducing waste and avoiding duplication, contributing to cost avoidance. Her transcribing of discharge prescriptions supports patient flow and timely bed availability.
