We are working in partnership with local County Borough Councils and Medequip Assistive Technology to bring you our Community Equipment Service.

Autism Service

The Integrated Autism Service has been set up jointly between Health and Social Care

Our Community Hospitals and Facilities are located throughout your local community

Rehabilitation support to adults in Gwent and Caerphilly after their discharge from hospital following stroke or brain injury.

The Community Nursing Service is central to providing high quality, comprehensive and accessible general nursing services

Gwent Frailty is a multi-disciplinary service within the Primary Care and Community Services Division

Mental Health Services within Aneurin Bevan University Health Board cover community and in-patient settings.

Due to the Health Board's response to the Covid-19 pandemic, we have made a number of changes to the physiotherapy service
