Living Well After Stroke Service (LWASS)

Who is the service for?

Our Living Well after Stroke Service offers some longer-term support to people and their families following stroke. The aim is to enable them to take control of their daily life, or support them to adjust to any ongoing difficulties they are experiencing.  

Support may involve: 

  • Education and advice about the effects of stroke 
  • Support to develop strategies to promote independence 
  • Support to adjust to or manage any ongoing difficulties being experienced 
  • Signposting to relevant NHS services or third sector organisations. 

Our LWASS service is accessible by anyone who lives in Blaenau Gwent, Caerphilly, Torfaen, Newport or Monmouthshire, who has experienced a stroke, or whose family member has experienced a stroke. 

 

Virtual and face to face support 

Most LWASS support is offered via telephone or online consultations using Attend Anywhere or MS Teams. Some home or community visits might also be recommended, depending on the support that is being offered.
