Why have I been referred to Lymphoedema?

Patients are referred via a Health Care Professional e.g. GP, or Community Nurse and have identified that you have swelling and have referred into our service for advice/support/assessment and management plan.

This will normally be prearranged whereby a clinician will contact you virtually to try and gather more information about your swelling. Following this, we will invite you to a face-to-face appointment if appropriate to complete our assessment, and provide further guidance and advice and care.  

You will be sent out a questionnaire (PROMS) for you to complete prior to your assessment with us. ABUHB and Lymphoedema Wales Clinical Network (LWCN) values what matters most to patients. Patients are the expert in their condition and are best placed to tell us how it affects them. The LYMPROM© (Lymphoedema-specific Patient Reported Outcome Measure) was developed to help adults with lymphoedema communicate their impact of lymphoedema. The information recorded by patients on LYMPROM© is explored during appointments and used by clinicians to support the right care at the right time for the individual patient. LYMPROM© can also help patients and staff together decide the focus treatment and to monitor progress. This is helping us as a service to provide a value-based service, this will mean patients can expect a lymphoedema service that focuses on what matters most to patients.
