The Big Conversation: Future Care Planning

At different points in our lives, we may want to consider what is important to us should there be changes in our health and wellbeing. Future Care Planning (FCP) is about thinking and planning ahead and can help people at any age or stage of life. FCP may involve conversations between individuals, those closest to them and healthcare professionals, giving people the opportunity to plan for future changes in their health and care. Although people’s experiences and views are personal to them, it is really important that what matters to each of us as individuals is known and respected.
 

 

 

 

We would like to invite anyone who is interested in, or have experience of, FCP. People’s feedback will be used to shape how we continue to raise awareness and improve support to those people and families who wish to consider a FCP.

We believe in the true value of feedback from diverse communities and would welcome attendance from individuals regardless of age, disability, sex, gender reassignment, sexual orientation, pregnancy and maternity, race, religion or belief and marriage and civil partnership.
