This service works on an open access basis and does accept self-referrals from patients and their families (with the patient’s consent) as well as from secondary care providers and GP practices (from either district/practice nurses or doctors).
You, your family or carer can self-refer by calling us and completing a form.
Please note: the 3 Day Bladder Diary form and questionnaire must be completed before a referral is agreed.

Before your appointment
You will be asked to complete and return a patient questionnaire along with a bladder or bowel diary. Following receipt of the questionnaire you will be offered an appointment.

Your face-to-face appointment
Please arrive 15 minutes before your appointment.
You'll see a specialist nurse at your face-to-face appointment.
Your phone appointment
If you have a phone appointment, the specialist nurse will aim to call you at the appointment time. Please note this time is approximate, they may call up to 30 minutes before or after. Please have your phone close to you during this time.
After your appointment
Our admin team will book any follow-up appointments you need.
We’ll write to you and your GP (or the doctor who referred you) with details of what we talked about at your appointment and details of the plan to follow before the next appointment.
Home Visit
For patients who are housebound, it may be more appropriate to complete a home visit, the nurse will contact you to arrange a suitable time and date.
