Gwent Frailty Service

Gwent Frailty is a multi-disciplinary service within the Primary Care and Community Services Division in Aneurin Bevan University Health Board and the five Gwent Local Authority Social Care Services, centred on providing patients with care and/or treatment closer to home and promoting patient independence.

The aims of the service are to:

• Reduce unnecessary hospital admissions by providing safe alternate pathways
• Minimising hospital stays by facilitating early hospital discharges

Opening Hours: 8:00am – 8:00pm, 7 days a week, 365 days a year
