Vaccination Appointment Cancellation Form

We are asking everyone to do everything they can to attend the first booster appointment they are offered. Having to rearrange an appointment could cause considerable delay to you receiving your booster vaccination, so please do everything you can to attend.

If you absolutely can't attend your appointment (including if you've recently contracted Covid-19), but would like to request a replacement appointment, please complete the following form so that we can ensure you are allocated a new appointment.

Please note that it will not be possible to issue a replacement appointment until the New Year due to the high number of residents requiring vaccinations at this time. Please be patient while you wait for us to contact you, and only fill in this form once. Submitting this form again will not speed up the process, but could in fact cause further delays in receiving your appointment.
