Annual Review and the 8 Care Processes

If you have had a diagnosis of Diabetes, you are entitled to an annual review check at your GP practice. During this check (which might span over two appointments), they will complete a set of routine tests and assessments recommended for managing diabetes to identify and prevent potential complications.
 

What to expect at your appointment
  • You will have a blood test which is important for a few things. We will check your HbA1C, kidney function and cholesterol levels.
  • You will also be expected to provide a urine sample which, in addition to the above, is crucial to detect early warning signs about your kidneys.
  • You will receive a foot check which looks for nerve damage (neuropathy), poor circulation (ischaemia), skin problems, and other risk factors.
  • Your blood pressure will be taken, along with your height and weight to review BMI.
  • And we will also have a discussion about lifestyle factors including smoking, exercise and healthy eating.

 

Regular testing and completion of these 8 care processes to monitor and manage type 2 diabetes can help to reduce the risk of complications and identify any complications earlier.
