Registrar Service

Once a cause of death has been agreed, the doctor will be able to complete the death certificate. This may then be sent to the Medical Examiner who may contact you to discuss the cause of death.  When this has happened, the Medical Examiner will send the death certificate to the Registrar’s office who will call you for an appointment to register your relatives death.

Once the death has been registered, the Registrars Service will issue you with a ‘green form’ (certificate for burial or cremation) which you then pass on to your chosen funeral director. The funeral director can them come to the hospital and transfer your relative into their care.

During the time your relative is resting with us you are welcome to contact the Care After Death Team on the above numbers to say your final goodbyes to your relative should you wish to do this.
