What if I am unable to answer the questions myself?

Whatever the reasons, we would still like you to tell us about how you are feeling. If you are unable to complete the assessment yourself, you may want to ask a family member, friend or carer to help you. If there is someone who will be able to assist you in completing the assessment, please make sure the answers you provide are your own, and not those of the person helping you.

If you are attending a clinic appointment, your healthcare worker may bring up the assessment on a laptop or mobile device. It is important that you are given the opportunity to complete the assessment without being observed by staff. However, a member of staff may demonstrate the use of the hand-held device to you, or a family member or friend, and answer any questions that you may have. If you require assistance completing the assessment, staff should be able to go through it with you.
