Our service is a self-referral service only and you can do this by filling out an online referral form or it can be completed by telephone, where a member of our team will help complete the form with you. Please call on 0300 303 4906 on Mondays, Tuesdays, Thursdays, or Friday mornings (9:00am - 1:00pm) or Wednesday afternoon (1:00 - 4:00pm).

If you would like to self-refer, you will need to know your height, current weight and have a list of your current medications. This form should take about 20-25 minutes to complete. In the end you will need to click submit.

 

Unable to attend? 

  • It is important that you contact us if your details or circumstances change.
  • If you are unable to attend an appointment, please contact the booking centre as soon as possible. This will allow us time to re-allocate your appointment. If you do not attend without informing us, you will be discharged.  
  • Please note we allow for 1 appointment to be rescheduled, if you are unable to attend more than 1 agreed appointment you may be discharged. 

 

Appointment & Booking 

For enquiries concerning appointments, waiting times and general queries, you can call the booking centre on 0330 0249301. The line is open between 8:30am – 16:30pm Monday to Friday except bank holidays. If you need to cancel an appointment, this number will be on any correspondence sent to you via post.

The email address for the generic inbox is ABB.WeightManagement@wales.nhs.uk. This email account is for patient data collection only and we are unable to respond to any queries via email. Please note that emails will not be checked on Fridays.


Please be aware we are not an urgent care or mental health service and therefore we are not able to respond urgently to queries. If you have concerns about mental health or require medical attention, please contact your GP.

We request that any clinical queries you have are kept for appointments with our clinicians or via telephone queries.

Please do not contact a member of the weight management service to check or re-arrange appointment times, this should continue to happen via the booking centre or by telephone if your appointment is with the Healthy Pregnancy Team.

We use team email accounts and therefore a number of people with have access to the information you send. If the information is intended for a particular individual please include in the subject ‘FAO: Staff members name’

To increase security, the number of people accessing our accounts is limited and we use password protected systems. However, with all accounts there is a small risk of the emails being intercepted so we will never ask you to provide or include personal details.

Please inform us at any time if you no longer wish to be contacted by email and we will update our records. This will not affect the level of service you receive.

Concerns 

If you have concerns, we encourage you to raise them at the time with your clinician. If this doesn't help or if you do not want to speak to staff who provided the service, then you can contact a member of the Aneurin Bevan University Health Board concerns team.  Further details about raising a concern and who to contact can be found on the Complaints & Concerns webpage

 

Telephone appointments 

If your appointment will take place via telephone, please ensure you have provided an up to date and preferred telephone number when booking your appointment. The therapist will attempt contact twice. If you do not pick up, the appointment will be cancelled and you may be discharged.

 

Attend Anywhere 

If you have chosen for your appointment to take place via video conference, the link will be sent in the appointment confirmation letter. If you do not receive this letter, please contact us and we will find an alternative method to send the link.

Microsoft Teams 

All of our group sessions are delivered via Microsoft Teams. Instructions for accessing the group will be emailed to you before the group start date. Please check your junk folder.
