For all group attendees, you will be sent, via your email that you provide on your self -referral form, a Microsoft teams link 7 days before joining with a support work book attached and possibly some questionnaires. The groups days and start times may vary and group sessions will last approximately 90 minutes on the same day each week. Please check your junk folder.
For limited individual appointments in Level 2 and Level 3 to see a Dietitian or counsellor or team with doctor you will receive a telephone call or Attend Anywhere appointment. This appointment time will range from 30-45 minutes.
At your first individual appointment with the Dietitian or counsellor or psychologist or Doctor, you will be asked why you have come to Adult Weight Management and what you hope to achieve. They will ask how your life is being affected, what you would like to change and what is most important to change right now. Sometimes it might not be the right time for you to commit to weight management for your own personal reasons. Please be honest about this and be aware that if you do not attend appointments without letting us know you will be discharged.
As a patient who accepts appointments to attend, you will have automatic acceptance of the therapeutic agreement. Details of this are set out below:

We would like to work in partnership with you, being partner’s means that we have a responsibility towards each other. This can only be achieved if we work together.
