If you are aged 18 years or over, clinically extremely vulnerable or clinically at risk, received your second dose more than three months ago and are yet to receive an offer of a booster appointment, please complete the form below to request your first booster dose, ensuring you provide all the information requested. Please do not complete this form if you are under 18 years old or have contracted Covid-19 within the past 28 days- you will be offered an appointment as soon as you are eligible.
Once we receive the information, we will check your previous vaccination record and if accurate, our team will contact you with details of your booster appointment within 72 hours.
