The Consultant Physician is responsible for the overall care of the person and will devise a treatment plan based on the individual’s needs. The physician will take into account all the medical aspects of care including medications, referral into other services, any tests and investigations required and co-ordinate the care within the MDT team. The Consultant will keep in touch with the GP and may request a change in medications.
