The Adult Weight Management Service is open and here to help you during and past COVID-19.
Our service is getting back to normal, however for now we are only providing our support with video calls, virtual groups and telephone calls. We are not offering face-to-face appointments for your safety and ours. Our level 3 MDT (Multi-disciplinary team) service has also now re-started.
How to Access Our Service
We have switched to self-referrals only as you are the best person to decide whether you would like our help. We also want you to be able to ask for our help when the time is right for you. You can self-refer to the Adult Weight Management Service online or by telephone by following the instructions below.
The easiest way to refer yourself is to fill in our online referral form.
You will need to know your height, current weight and have a list of your current medication. The form should only take 5-10 minutes to complete. On the last page you will need to click ‘submit’.
The fastest way to refer yourself is by using our online form, but if you are unable to do that, then you can refer yourself by telephone. The call will take around 15-20 minutes and a member of our team will ask you a few questions about you and your weight. You will need to know your height, current weight and have a list of your current medication.
Please call us on 01633 431 776 on Tuesdays or Thursdays between 09:00am and 12:30pm to refer by telephone.
