Community Neurological Rehabilitation Service

Aneurin Bevan University Health Board’s Community Neurological Rehabilitation Service offers rehabilitation support to adults in Gwent and Caerphilly after their discharge from hospital following stroke or brain injury, and for psychological support of non-epileptic seizures.

The effects of a stroke or brain injury can be really difficult to come to terms with and learn to live with. We work with people affected by stroke or acquired brain injury to understand their condition, develop and re-learn skills, return to hobbies or employment, optimise quality of life, and increase independence and self-management in the community.

  • Therapy Assistant Practitioners

 

What do we do?
 
  • Person Centred, Multidisciplinary team approaches to rehabilitation, helping people to identify and work toward goals that are important to them, to help maximise their independence after a stroke or brain injury. 
  • Individual and group-based interventions.
  • An allocated team of clinicians to provide and monitor rehabilitation in the home, in hospital outpatient departments, or in the community. 
  • We also work closely with inpatient teams across Aneurin Bevan and Cardiff & Vale hospitals to support smooth transitions for people returning home.
