Care After Death and Bereavement

Aneurin Bevan University Health Board offers our sincere condolences on the death of your loved one. When someone close to you has died it can be immensely difficult to work through the practicalities of what needs to be done and who needs to be informed.

Following a death in hospital, the Care After Death Team will help to arrange the death certificate needed for you to register the death and arrange the funeral.  

Following the death of your relative, please contact the Care After Death Team when you are ready. Please telephone 01443 802406 and select the option depending on which hospital your relative died in:

  • Option 1 – Grange University Hospital
  • Option 2 – Royal Gwent Hospital, St Woolos Hospital and County Hospital
  • Option 3 – Nevill Hall Hospital and Chepstow Community Hospital
  • Option 4 – Ysbyty Ystrad Fawr and Ysbyty Aneurin Bevan
