The aim of the Nurse led Heart Failure service is to support patients to manage their condition and help recognise and manage your symptoms. Specialist nurses will review medication regularly, which may be adjusted from time to time depending on the patients condition and symptoms.
At the first appointment with the specialist nurse a suitable treatment plan option is identified and discussed with you. This plan may include aspects other than medication alone, e.g. cardiac rehabilitation including exercise, education on how your diet and lifestyle can impact your condition or symptoms.
