How to access support from the weight management service

We are currently only accepting referrals for people with a Body Mass Index (BMI) of 35 and over, who have at least one of the conditions that meets our urgent criteria. Referrals will be accepted from GPs, hospital consultants (Doctors) and via our self-referral form.

 Below provides guidance on conditions that may be accepted under our urgent referral criteria, however each referral will be considered on a case by case basis.

  • Cancerous or pre-cancerous conditions in which weight loss would improve outcomes or aid access to therapies
  • Patients requiring urgent weight loss for organ transplant
  • Idiopathic intracranial hypertension (IIH) requiring frequent lumbar punctures and/or with visual compromise (likely to lose sight)
  • Proven genetic cause of obesity and not eligible for Setmelanotide (Imcivree®).
  • Weight loss required for assisted conception (IVF) in women under the care of a fertility services, in cases where weight loss would be beneficial in women age 35 years and over.
  • Other conditions such as a severe lung, liver or heart condition that is continuing to get worse despite treatment or treatment cannot be provided until weight loss is achieved) may be considered on a case-by-case basis.

You do not need to request an appointment with a hospital doctor or GP to make a referral. To meet one of the criteria above you will already be under the care of a hospital doctor and will be able to discuss a referral to the weight management service at your next appointment or can complete the self-referral form below.

Children and Young People under 18 years can be referred to 'Connect' Child and Young Person Weight Management Service - Aneurin Bevan University Health Board
